QUESTIONNAIRE 5 - for NHS or Community Based Integrated Medicine




Section 5 of the Directory is open to NHS or community based integrated medicine services including:

  • NHS or community based Integrated Medicine Services offering one type of therapy, self-help or support;
  • NHS or community based Integrated Medicine Services offering specialist help for a specific medical condition;
    (IM services can include those providing complementary and alternative medicines, psychological and spiritual support or self-help classes)

You may register for the Directory using a short form or a long form. Your entry on the Healthy Bristol website will appear the same whichever you do, but if you do the long form, this will be available to Bristol and Bath doctors to help them make their referral decisions.
We strongly recommend that you use the long form as it will not only increase the likelihood of referrals, but thinking about the questions will also help you with your professional development and help you to improve your practice to reflect the new clinical governance framework which is becoming required within UK healthcare.
